8/15/2023 EnchantaVaughn Approved Behavior: When QMHP-C arrived at Laylah’s residence, her observation of Laylah was exhibited as angry, hostile, and irritable, as it was reported by the mother that Laylah was not having a good day. Laylah displayed angry outbursts and lashed out at QMHP-C when inquired about why she was having a bad day. Behavior: Laylah showed unwillingness to engage in the daily activities provided to her during the session. Laylah was seen as distant and lost in her own feelings and thoughts, as she displayed disinterest and nonchalant attitude. Laylah began to exhibit a persistent failure to comply to rules and expectations within the session. Behavior: Laylah appeared to be in a bad mood evident by her facial expression and desire to not participate in session. As time progressed, Laylah became more active throughout session and her mood became to improve. Intervention: QMHP-C first revisited with Laylah the use of muscle relaxation, pace breathing, calming imageries, and self-control techniques, to help control her anger behaviors. QMHP-C demonstrated and provided positive acceptance, as she attempted to reestablish a mutual and trusting relationship with her. Intervention: QMHP-C processed with Laylah to gain a better understanding and identify stimulus that triggered her angry emotions. QMHP-C reviewed with Laylah conflict resolution skills through instruction, modeling, role-playing, and behavioral rehearsal. QMHP-C provided mom with at home strategies to practice with Laylah in case the behaviors that persists become uncontrollable or overwhelming. Intervention: QMHP-C taught Laylah communication and assertiveness skills when interaction with others. QMHP-C defined non-verbal communication as she can communicate without words such as body language and eye contact as Laylah struggles with expressing herself. QMHP-C engaged Laylah in calming skills to relax when triggered by others. QMHP-C engaged Laylah in a role-played demonstrating to Laylah different techniques to demonstrate appropriate ways to show feelings to avoid aggressively acting out. QMHP-C discussed values and beliefs with Laylah and identified her feelings and thoughts when someone disrespects her. QMHP-C role-played scenarios with Laylah on how she should display acceptable behaviors toward her mother and authority figures whether in the home, community and school. Response: Laylah displayed difficulties, as she failed to consider the calming consequences of her actions and behaviors. Laylah presents irresponsible, attention-seeking, and aggressive behaviors so that she would be left alone. Laylah appeared careless, as she shrugged her shoulders at QMHP-C indicating she didn’t care. Response: Laylah displays a poor relationship with QMHP-C, as evidenced by her turning her back and avoiding eye contact. Laylah continued to have problems managing her anger. Laylah displayed inappropriate verbal and non-verbal communication throughout session. Response: Laylah appeared to be receptive to QMHP-C’s interventions and services that were provided to her during the session. Laylah continues to struggle with communicating effectively with her family members as he shuts down in lieu of discussing her feelings and thoughts age-appropriately. Laylah shared that she is trying to do better when it comes to her and her mother but that is a work in progress because her mother has a smart mouth towards her although she is her mother.
